Below you will find all the necessary enrollment forms for preschool. All students will need to have complete sets of forms on file in the preschool office. This completed paperwork will be turned in to the preschool on or before August 29th.
Forms can be Scanned and emailed to mpedersen@fcpcl.org
Forms can also be filled out and dropped off during FCPCL Business Hours.
