Would you like to receive First Church E-News?YesNo Field Trip Permission *FIRST CHURCH PRESCHOOL WALKING FIELD TRIP PERMISSION During the school year, First Church Preschool classes may participate in walking field trips around the church campus, to the library, or to other local businesses and community resources. Walking field trips will relate to the unit of study the school is exploring. Walking field trips are a beneficial way to expose children to local resources and relationships within our community. Your expressed permission is required for your child to participate in a walking field trip. Parents will be informed in advance of any walking field trip. By signing below, you permit your child to accompany their class on walking field trips as planned and supervised by First Church Preschool. Staff will ensure a safe walking route and safe ratios/supervision to and from school. Field Trip Permission Discipline Policy *FIRST CHURCH PRESCHOOL GUIDANCE AND DISCIPLINE POLICY In early childhood education classes, challenging behavior occurs from time to time. During the first five years of life, children are just beginning to learn how to handle their own intense emotions and conform to the behavior expectations of society. As parents know, this can be a long and challenging process. At First Church Preschool, we define challenging behavior as any behavior that: - Interferes with children's learning, development, and success at play and learning. - Is harmful to the child other children, or adults. - Puts a child at high risk for later social problems or school failure. Such behavior can be direct (e.g. hitting, pushing, biting, kicking) or indirect (e.g. teasing, ignoring rules and instructions, excluding others, name-calling, destroying objects, and having temper tantrums.) At First Church Preschool, we teach children to handle their emotions and behavior by: - Teachers modeling appropriate and positive behavior. - Designing the physical environment to minimize conflict. - Maintaining age-appropriate expectations for children's behavior. - Closely observing and supervising children's activities and interactions. - Helping children verbalize their feelings, frustrations and concerns. - Removing a child that endangers the other children to a supervised space where they can regain composure. - Discipline and/or guidance will be positive, productive, and immediate when behavior is inappropriate. When a pattern of behavior persists that endangers, self, others, or property or significantly disrupts the program, we will work with a child's family to find solutions, up to and including referral for outside services, suspension, or exclusion from the First Church Preschool program. I have, read, understood, and approved of the policy stated above. Discipline Policy Liability Waiver *LIABILITY WAIVER I certify that my child is in good health and physical condition and does not suffer from any undisclosed condition or disability that may prevent or limit his participation in all activities conducted by First Church Preschool. I acknowledge and agree that First Church Preschool/First Church shall not assume any responsibility or liability for personal injury or damage caused by any injury my child may sustain and, further, that I will hold First Church harmless for any such injury. I assume all risks, whether known or unknown, of my child's injuries. If First Church Preschool cannot reach a parent, guardian, or emergency contact, I permit my child to be transported to the nearest hospital for treatment in the event of an accident or emergency. I hereby further authorize any staff or employee to provide for, approve, and allow health care at the hospital. I further agree that the foregoing waiver indemnity, assumption of risk, and release are intended to be as broad and inclusive in favor of First Church and its preschool as permitted by the laws of the State of Illinois. Singular words include plural, and the use of one gender includes the other. I have read, understood, and approved the waiver stated above.
